Dihexa vs Zilucoplan (Zilbrysq)

An educational, source-based comparison of Dihexa and Zilucoplan (Zilbrysq) — how each peptide works, what it's researched for, and what to know before going deeper.

Nootropic · Synaptogenesis
Dihexa

Angiotensin IV analog researched for synaptogenesis.

Mechanism

A hexapeptide derived from angiotensin IV that crosses the blood-brain barrier and potentiates hepatocyte growth factor (HGF) / c-Met signaling, associated with new synapse formation in preclinical models.

Research areas
  • Synaptogenesis and dendritic spine density
  • Alzheimer's and cognitive aging models
  • Memory consolidation in rodents
Considerations
  • No human clinical trial data; theoretical proliferation concerns via HGF/c-Met.
  • Not FDA-approved.

Complement Inhibitor · Neuromuscular
Zilucoplan (Zilbrysq)

Macrocyclic peptide complement C5 inhibitor for generalized myasthenia gravis.

Mechanism

A synthetic 15-amino-acid macrocyclic peptide that binds complement component C5 and blocks its cleavage to C5a/C5b, preventing formation of the membrane attack complex at the neuromuscular junction. Self-administered subcutaneously.

Research areas
  • Generalized myasthenia gravis (anti-AChR antibody positive)
Considerations
  • FDA-approved 2023.
  • Requires meningococcal vaccination — risk of serious meningococcal infection.
  • Injection-site reactions common.
