Motixafortide (Aphexda) vs Sincalide (Kinevac)

An educational, source-based comparison of Motixafortide (Aphexda) and Sincalide (Kinevac) — how each peptide works, what it's researched for, and what to know before going deeper.

CXCR4 Antagonist · Hematology
Motixafortide (Aphexda)

CXCR4 antagonist peptide that mobilizes stem cells for autologous transplant in multiple myeloma.

Mechanism

A long-acting synthetic peptide antagonist of the CXCR4 chemokine receptor. Disrupts the CXCR4/CXCL12 axis anchoring hematopoietic stem cells in the bone marrow, releasing them into circulation for collection. Used with G-CSF.

Research areas
  • Stem cell mobilization for autologous transplant in multiple myeloma
Considerations
  • FDA-approved September 2023.
  • Common side effects: injection-site reactions, flushing, pruritus.
  • Risk of hypersensitivity / anaphylactoid reactions.

CCK Analog · Diagnostics
Sincalide (Kinevac)

Synthetic C-terminal octapeptide of cholecystokinin for gallbladder imaging.

Mechanism

Synthetic CCK-8 that binds CCK-A receptors on gallbladder smooth muscle, triggering contraction; used to assess gallbladder ejection fraction in cholescintigraphy.

Research areas
  • Gallbladder ejection fraction (HIDA scan)
  • Pancreatic secretion testing
  • Small bowel transit imaging
Considerations
  • FDA-approved.
  • Abdominal cramping, nausea, diarrhea common; contraindicated in suspected gallstone obstruction.
